BIRTH CERTIFICATE - Actual certificate, copies or hospital birth record is not valid.
IMMUNIZATION RECORDS - All shots must be given prior to the first day of school.
Immunizations are available through your family doctor or through the Macomb County Health Department. For more information go to http://www.macombcountymi.gov/publichealth/clinicservices/fhs_immunization_clinic.htm
TWO PROOFS OF RESIDENCY - Driver's license of parent/guardian, lease agreement, mortgage, utility bill, and/or property tax statements.
OTHER LEGAL DOCUMENTS - Custody or guardianship paperwork, foster care paperwork... All documents must have current dates and signatures.
UNOFFICIAL COPY OF STUDENT TRANSCRIPT - If entering 9th grade OR a copy of the final 8th grade report card will suffice.
IEPC - If your child receives special education services, the most recent copy of their IEPC is required.
Any student entering MCHS in the middle of a card marking must supply: withdrawal grades, current report card, attendance and disciplinary records from their previous school.
