How to Enroll

Visit the school your child will  attend to complete the enrollment process.

Grades PreK-5: Seminole Academy, 1500 Mulberry, Mount Clemens, MI 48043
Grades 6-8: Mount Clemens M.S., 155 Cass Avenue, Mount Clemens, MI 48043
Grades 9-12: Mount Clemens H.S., 155 Cass Avenue, Mount Clemens, MI 48043

Each school has its' packet available for download. You can also pick them up in the Main Office of each school.

  • Birth Certificate - Actual certificate, copies or hospital birth record is not valid, and enrolling parent's Driver's License or Photo ID.
  • Immunization Records - All shots must be given prior to enrollment. Immunizations are available through your family doctor or through the Macomb County Health Department. 
  • Required Vaccines
  • Two Proofs of Residency -  Lease agreement, mortgage, utility bill, and/or property tax statements.
  • Other Legal Documents -  Custody or guardianship paperwork, foster care paperwork.  All documents must have current dates and signatures.
  • Hearing and Vision Screening - For New Kindergarten Students Only
  • Free Vision and Hearing Screening is available for children ages 3 - 18 at the Macomb County Health Department, call 586 412-5945 for more information.

  • Unofficial Copy of Student Transcript - If entering 9th grade OR a copy of the final 8th grade report card.
  • Any student entering MCHS in the middle of a card- marking period must supply the following: withdrawal grades, current report card, attendance records, and disciplinary records from his or her previous school.
